Panchkula: Inform Municipal office within 21 days of birth or death

Residents of the Municipal Corporation area in Panchkula are required to inform the Sector 4 office of the Municipal Corporation within 21 days of any births or deaths in their households. The office will provide a certificate free of cost upon application within 21 days. If the information is provided between 21 to 30 days, senior officers of the corporation will handle it, while if it is after 30 days, the Civil Surgeon will verify the details before issuing the certificate. In cases where the information is provided after one year, the certificate will be issued only after verification by the DC.
To obtain a birth certificate for a newborn, parents should visit the Corporation Sector-4 office with their Aadhaar card and fill out Form No. 1 and the birth application form. The forms should be verified by the Anganwadi and area councilor before being submitted to the municipal office. The birth certificate will be issued within a week. For a death certificate, the applicant should visit the municipal office with their own certificate and the deceased’s certificate and fill out Form No. 2. The forms should be verified by the crematorium before being submitted to the municipal office. The death certificate will be issued within a week to 15 days.
It is important for residents to comply with these regulations to ensure that accurate records are maintained and that certificates are issued in a timely manner. The Municipal Corporation should also consider introducing measures to simplify the application process and make it more accessible to residents
